Small flexible fittings like this are easy to tear, harden or lose when the tank is washed out or the hose is pulled off. A stopper that has split, shrunk or no longer grips firmly should be replaced.
Press the new one fully home and make sure it sits square. Inspect the external hose at the same time, since the two work together.
